Forms for Rockin’ Recipients

When you receive a tuition statement or you’ve paid your tuition bill, submit whichever payment form is right for your needs:

• Scholarship Payment Form —  Submit this form if you’d like Ruby’s Rainbow to send your scholarship check directly to your school.
• Scholarship Reimbursement Form — Submit this form if you or your parent/legal guardian have already paid your tuition and you need to be reimbursed.

NOTE: If you submit a Payment or Reimbursement form and don’t hear back from Catherine within three business days, please email catherine@rubysrainbow.org to make sure your form was received!
